A short report, built from what your patient logged

Parkinson Tracker is a free journal for iPhone and iPad. Before the appointment, the patient creates a PDF report and chooses to send it to you. The report describes; it does not interpret.

What the report contains

Page 1, summary
What the patient wants to discuss and what went well, in their own words. The time spent in each logged state (Good, Slow, Stuck, Dyskinesia), with the number of days with entries. Logged OFF episodes. When available: the daily check-in, logged doses, sleep and steps. Declared treatments and their changes.
Page 2, day by day
A grid per day and per two-hour slot, in colour and with patterns, readable printed in black and white. The pump, if used: declared boosts, estimated continuous dose.
Last pages, details and methods
Times of logged doses against scheduled times. Sleep and activity. Deep brain stimulation, if used. Methods and definitions.

What the report does not contain

No score, no coverage percentage, and no remark about missed doses on page 1. A section appears only if it has data.

A complete sample

A sample PDF report, with made-up data, can be sent to you. Write to contact@parkinsontracker.ch.

What the data are worth, and what they are not

Declared
Entered by the patient. It is a feeling noted at the moment they choose, not a measurement. The report depends on what they thought to log.
Observed
Imported from Apple Health with the patient’s consent: steps, sleep, workouts. Quality depends on the measuring device.
Estimated
Calculated from other data, for example a pump’s continuous dose from the declared events.

The three are never merged. A period without entries stays empty: it is neither counted as a state nor replaced by zero. A link in time is never presented as a cause.

Parkinson Tracker is a personal journal. It does not diagnose, it makes no recommendations and it does not replace a doctor’s advice.
